Faculty and Staff Forms

Admission to Candidacy Form

Use this form when one of your doctoral students is ready to change classification and progress into the final research-and-writing phase of your doctoral degree program. This form is only available through the Graduate Information Management System (GIMS), which you can access by clicking on this link: GIMS.

Log in to GIMS, click on the "Student Records and Supervisory Committee Records" button, enter the student's UFID, click on the "Graduate School Forms" button at the bottom of his/her record screen, and then click on the "Candidacy Form" button on the menu of forms that appears. GIMS will automatically fill in most of the information for you and guide you through the rest of the process.

Change of Degree Form

Use this form to change a UF graduate student's classification (major, department or college), add another degree program segment to his/her record (for example, a doctorate along with a master's degree), or admit a current UF graduate student in another program into your program. This form is only available through the Graduate Information Management System (GIMS), which you can access by clicking on this link: GIMS.

Log in to GIMS, click on the "Student Records and Supervisory Committee Records" button, enter the student's UFID, click on the "Graduate School Forms" button at the bottom of his/her record screen, and then click on the "Change of Degree Form" button on the menu of forms that appears. GIMS will automatically fill in most of the information for you and guide you through the rest of the process.

Concurrent Degree Program Form

Use this form to admit a UF student already in another graduate program into your graduate program, so he/she can pursue both degrees at the same time. Attach a Change of Degree Program Form to this form when you submit it.

Final Missing International Credentials Hold Petition

Use this form to request a second, final lifting of a registration hold for a student with missing credentials from a non-US institution.

Final Defense/Final Examination Form

Use this form to collect supervisory committee signatures and document the final result of a graduate student's final defense of his/her thesis or dissertation, or the final examination required by his/her degree program. This form is only available through the Graduate Information Management System (GIMS), which graduate coordinators and graduate staff can access by clicking on this link: GIMS.

Log in to GIMS, click on the "Student Records and Supervisory Committee Records" button, enter the student's UFID, click on the "Graduate School Forms" button at the bottom of his/her record screen, and then click on the "Final Exam Form" button on the menu of forms that appears. GIMS will automatically fill in most of the information for you and guide you through the rest of the process.

GIMS Access Authorization and Password Request

Use this form to request a faculty/staff member user ID and password so you can access student and faculty records on the GIMS (Graduate Information Management System) website.

Graduate Coordinator Appointment Form

Use this form to appoint a new or addditional graduate coordinator for your department and get him/her put on the UF Graduate School's roster of contacts and e-mail listserv.

Graduate Credit Transfer Form

Use this form to transfer graduate credits from a UF student's prior graduate degree (elsewhere or at UF) and apply them to your program's degree requirements.

Graduate Faculty Status Appointment Form

Use this form to get departmental graduate faculty status for a faculty member. Attach a printout of his/her PeopleSoft Employment Screen if he/she is an "affiliate appointment" (appointed, paid and accruing tenure in one unit, but also teaching, supervising students and doing research in another unit without being paid from the second unit's budget) or "courtesy appointment" (appointed to a unit without being paid from its budget).

Joint Degree Program Form

Use this form to admit a UF student already in a UF professional program (dentistry, law, medicine, pharmacy or veterinary medicine) into your graduate program, so he/she can pursue both degrees at the same time.

Nontraditional Doctoral/Master's Degree Program Form

Use this form to set up and approve a study program for a UF doctoral student who wants to earn a master's degree outside of his/her doctoral department at the same time.

Special Supervisory Committee Appointment Form

Use this form to put someone who does not have UF graduate faculty status on a student's supervisory (thesis or disseration) committee.

Supervisory Committee Form

All supervisory (thesis or dissertation) committees are to be entered online through the GIMS (Graduate Information Management System) website.

Traveling Scholar Form

Use this form if you have a student who wants to participate in the UF Traveling Scholar Program as a UF graduate student attending an institution of higher learning elsewhere, either within the Florida State University System or outside of the Florida State University System - but within the United States.
